Complete this form if you are two or more years away from starting your pension and want to increase the survivor pension for your spouse.

Complete this form and return it to OPTrust if you are an active, deferred or divested member of the OPSEU Pension Plan and you wish to update your spousal information and/or designate a beneficiary before you start receiving a pension.
Statement of Marital Status
Complete this form and return it to OPTrust if you are a member of the OPSEU Pension Plan and your spousal status has changed.
Certificate of Health
Complete this form if you want to provide a survivor pension for a post-retirement spouse more than 90 days after acquiring a new spouse in retirement.
Application to Contribute for an Unpaid Leave of Absence
This form is to be completed by members for each leave of absence.
Non-Ontario Public Service Pension Service and Earnings Information
Please contact your previous employer and/or pension plan administrator to complete Sections 2, 3 and 4.
Medical Examination Report
This form should be completed by a physician or specialist as part of the process of applying for disability benefits from the OPSEU Pension Plan.
Application for Lump Sum Payout Due to Shortened Life Expectancy
Complete this form if you wish to apply to withdraw the lump sum value of your pension from the Plan because you have an illness or physical disability.
Declaration of Attendance at a Secondary or Post-Secondary Institution
Complete this form if you are the eligible child of a deceased member of the Plan and you need to confirm you are a student.
